I’m a little confused about when it is used and when it is not, what are the rules surrounding its use?
Lots of rules, even I don’t have the patience to type them up. You should ask your teacher because I don’t think it’s possible you could ‘get’ it just from reading online explanations.
The generally use is to express uncertainty, doubt, regret or wish.
IE "J’aimerais que tu sois plus dicipliné." I wish you were more disciplined or I would like it if you were more disciplined.
The imparfait, passé, and plus-que-parfait are almost never used except in extremely complicated literary texts. They are for: replaces the subjontif present for a text in the past, expresses an action that has already happened before another action mentioned in the same sentence, replaces the subjontif passé in the part of a sentence’s statement that happened before the other part of the sentence, respectively.
Most people just "get it" naturally from reading books and assuming what it means, without using the dictionary definitions.

For example, por lo general is a trigger for the imperfect. What are trigger words for the subjunctive? Does the sentence also have to begin with a verb + que, or are they other ways of structuring the sentence?
You must use the subjunctive after :
- ojalá
ojalá lo traiga!
- para que
lo digo para que lo sepa
- siempre que
lo compraré siempre que tenga dinero
- como si
lo exige como si fuera suyo
But most of the time it’s the verb that implies a subjunctive.
queremos que te portes un poco mejor
(querer = will)
siento que no venga
(sentir = regret)

Does the subjunctive mood have to be expressed in the form of a wish ("I wish I were president"), or a conditional statement that includes a premise and a conclusion ("If I were president, the White House would be pink.")?
I meant, is there any OTHER way of expressing the subjunctive mood, besides a conditional and a wish statement?
Either one is subjunctive mood.
